Ammonia Leak results in 1 fatality

A rupture in a high pressure ammonia line at a North Carolina chicken processing facility resulted in one employee fatality:

Ammonia is frequently used as a refrigerant in the food industry. Facilities containing a 10,000 pound inventory of ammonia are covered by OSHA’s Process Safety Management (PSM) standard: 1910.119. The PSM standard contains 14 required elements. One element, process hazard analysis, requires an evaluation of all processes covered by the PSM standard to identify, evaluate, and control the hazards involved in the process. Recommendations for improvement must be promptly addressed (typically 1 – 2 year time frame.) In addition, the process hazard analysis must be updated every 5 years.
